তবে শুধু ACL ইনজুরির গ্রেড দেখেই চিকিৎসা নির্ধারণ করা হয় না। রোগীর উপসর্গ, ফাংশনাল ইনস্ট্যাবিলিটি, হাঁটুর অন্য কোনো ইনজুরি, খেলাধুলার চাহিদা এবং রোগীর রিকভারি গোলও বিবেচনা করতে হয়।
ACL ইনজুরির সাধারণ উপসর্গ
ACL ইনজুরির উপসর্গ রোগীভেদে ভিন্ন হতে পারে। তবে সাধারণত নিচের উপসর্গগুলো দেখা যায়:
- পপ শব্দ বা অনুভূতি: ইনজুরির সময় অনেকেই হাঁটুর ভেতরে "পপ" শব্দ শুনতে বা অনুভব করতে পারেন।
- দ্রুত হাঁটু ফুলে যাওয়া: ইনজুরির পর তুলনামূলক অল্প সময়ের মধ্যেই হাঁটু ফুলে যেতে পারে।
- হাঁটুতে ব্যথা: দাঁড়ানো, হাঁটা বা খেলা চালিয়ে যাওয়া কষ্টকর হতে পারে।
- হাঁটুর অস্থিরতা বা গিভিং ওয়ে: বিশেষ করে ঘোরা, পিভটিং বা দ্রুত দিক পরিবর্তনের সময় হাঁটু অনির্ভরযোগ্য মনে হতে পারে।
- রেঞ্জ অব মোশন কমে যাওয়া: ব্যথা ও ফোলার কারণে হাঁটু পুরোপুরি সোজা বা ভাঁজ করতে অসুবিধা হতে পারে।
- ওজন নিতে অসুবিধা: ইনজুরির প্রাথমিক পর্যায়ে আক্রান্ত পায়ে ভর দিয়ে হাঁটা কষ্টকর হতে পারে।
- হাঁটুর ওপর আত্মবিশ্বাস কমে যাওয়া: ব্যথা কমে গেলেও দৌড়ানো, লাফানো বা দ্রুত দিক পরিবর্তনের সময় অনেক রোগী হাঁটুকে বিশ্বাস করতে পারেন না।
মোচড়জনিত বা স্পোর্টস ইনজুরির পর হঠাৎ হাঁটু ফুলে যাওয়া, পপ অনুভূতি এবং হাঁটুর অস্থিরতা দেখা দিলে সঠিকভাবে অ্যাসেসমেন্ট করা উচিত। তবে অন্যান্য হাঁটুর ইনজুরিতেও একই ধরনের উপসর্গ দেখা দিতে পারে।
ACL ইনজুরির সাধারণ কারণ
যেসব মুভমেন্টে হাঁটুর ওপর বেশি রোটেশনাল ফোর্স বা হঠাৎ ডেসেলারেশন তৈরি হয়, সেসব অবস্থায় ACL ইনজুরি বেশি হতে পারে।
সাধারণ ইনজুরি মেকানিজমের মধ্যে রয়েছে:
- পা মাটিতে স্থির থাকা অবস্থায় হঠাৎ দিক পরিবর্তন করা।
- দৌড়ানোর সময় খুব দ্রুত গতি কমানো বা থেমে যাওয়া।
- জাম্পের পর ভুলভাবে ল্যান্ড করা।
- হাঁটু পিভট বা টুইস্ট করা।
- হাঁটুর আশেপাশে সরাসরি সংঘর্ষ বা আঘাত।
- স্পোর্টস-স্পেসিফিক মুভমেন্টের সময় লোয়ার লিম্বের নিয়ন্ত্রণ হারানো।
যেসব খেলায় বারবার কাটিং, পিভটিং, জাম্পিং এবং দ্রুত অ্যাক্সেলারেশন বা ডেসেলারেশন করতে হয়, সেসব খেলায় ACL-এর ওপর তুলনামূলক বেশি চাপ পড়ে।
ACL ইনজুরির রিস্ক ফ্যাক্টর
ACL ইনজুরি সাধারণত কোনো একটি কারণের জন্য হয় না। অ্যাক্টিভিটির চাহিদা, মুভমেন্ট প্যাটার্ন এবং ব্যক্তিগত বিভিন্ন ফ্যাক্টর একসঙ্গে ঝুঁকি বাড়াতে পারে।
রিস্ক ফ্যাক্টরের মধ্যে রয়েছে:
- নিয়মিত পিভটিং, কাটিং বা জাম্পিং করতে হয় এমন খেলায় অংশগ্রহণ।
- ল্যান্ডিং বা দিক পরিবর্তনের সময় দুর্বল নিউরোমাসকুলার কন্ট্রোল।
- পূর্বে ACL বা উল্লেখযোগ্য হাঁটুর ইনজুরি থাকা।
- লোয়ার লিম্বের অপর্যাপ্ত শক্তি বা মুভমেন্ট কন্ট্রোল।
- হাঁটুর বায়োমেকানিক্সকে প্রভাবিত করতে পারে এমন অ্যানাটমিক্যাল ফ্যাক্টর।
- ট্রেনিং পরিবেশ, খেলার মাঠের সারফেস অথবা ফুটওয়্যার-সারফেস ইন্টারঅ্যাকশন।
কিছু পিভটিং স্পোর্টসে নারী অ্যাথলেটদের মধ্যে নন-কন্টাক্ট ACL ইনজুরির হার পুরুষদের তুলনায় বেশি দেখা যায়। এর পেছনে অ্যানাটমিক্যাল, বায়োমেকানিক্যাল এবং অন্যান্য বিভিন্ন ফ্যাক্টর নিয়ে গবেষণা হয়েছে। তাই এই পার্থক্যকে একটি মাত্র কারণ দিয়ে ব্যাখ্যা করা ঠিক নয়।
ACL ইনজুরি কীভাবে নির্ণয় করা হয়?
ACL ইনজুরি নির্ণয়ের শুরুতেই সাধারণত কীভাবে ইনজুরি হয়েছে এবং এরপর কী ধরনের উপসর্গ হয়েছে তা বিস্তারিতভাবে জানা হয়। এরপর হাঁটুর ক্লিনিক্যাল অ্যাসেসমেন্ট করা হয়।
একজন ফিজিওথেরাপিস্ট, স্পোর্টস মেডিসিন ফিজিশিয়ান বা অর্থোপেডিক স্পেশালিস্ট সাধারণত মূল্যায়ন করতে পারেন:
- ব্যথা ও ফোলা।
- হাঁটুর রেঞ্জ অব মোশন।
- হাঁটা এবং পায়ে ওজন নেওয়ার সক্ষমতা।
- হাঁটুর স্ট্যাবিলিটি।
- মাসল অ্যাক্টিভেশন ও স্ট্রেংথ।
- ফাংশনাল লিমিটেশন।
- মেনিস্কাস বা হাঁটুর অন্যান্য স্ট্রাকচার ইনজুরির সম্ভাবনা।
অ্যাসেসমেন্টের অংশ হিসেবে ল্যাকম্যান টেস্ট, অ্যান্টেরিয়র ড্রয়ার টেস্ট এবং পিভট-শিফট টেস্ট-এর মতো স্পেশাল টেস্ট ব্যবহার করা হতে পারে।
ACL ইনজুরিতে এমআরআই কি প্রয়োজন?
সন্দেহজনক ACL ইনজুরি কনফার্ম করা এবং মেনিস্কাস, কার্টিলেজ বা অন্যান্য লিগামেন্টে ইনজুরি আছে কি না মূল্যায়নের জন্য এমআরআই প্রায়ই ব্যবহার করা হয়।
তবে শুধু এমআরআই রিপোর্টের ওপর ভিত্তি করে সিদ্ধান্ত নেওয়া উচিত নয়। ইনজুরির ইতিহাস, রোগীর উপসর্গ এবং ক্লিনিক্যাল অ্যাসেসমেন্টের সঙ্গে এমআরআই ফলাফল মিলিয়ে দেখা গুরুত্বপূর্ণ।
এক্স-রে ACL দেখাতে পারে না। তবে ফ্র্যাকচার, ডিসলোকেশন বা অন্য কোনো হাড়ের ইনজুরি সন্দেহ হলে এক্স-রে প্রয়োজন হতে পারে।
সব ACL ইনজুরিতেই কি অপারেশন প্রয়োজন?
না। ACL টিয়ার হয়েছে মানেই প্রত্যেক রোগীর সার্জারি লাগবে, এমন নয়।
কিছু রোগী স্ট্রাকচার্ড নন-অপারেটিভ রিহ্যাবিলিটেশনের মাধ্যমে ভালো ফাংশন ফিরে পেতে পারেন। বিশেষ করে যাদের হাঁটু দৈনন্দিন কাজের সময় ফাংশনালি স্ট্যাবল থাকে এবং যাদের কাজ বা খেলাধুলায় নিয়মিত হাই-ডিমান্ড পিভটিং প্রয়োজন হয় না, তাদের ক্ষেত্রে নন-সার্জিক্যাল ম্যানেজমেন্ট বিবেচনা করা যেতে পারে।
নন-সার্জিক্যাল চিকিৎসার মধ্যে থাকতে পারে:
- ব্যথা ও ফোলা নিয়ন্ত্রণ।
- হাঁটুর স্বাভাবিক মুভমেন্ট ফিরিয়ে আনা।
- প্রগ্রেসিভ কোয়াড্রিসেপস ও হ্যামস্ট্রিং স্ট্রেংথেনিং।
- হিপ ও লোয়ার লিম্ব স্ট্রেংথেনিং।
- ব্যালেন্স ও নিউরোমাসকুলার ট্রেনিং।
- ফাংশনাল রিহ্যাবিলিটেশন।
- ধীরে ধীরে উপযুক্ত অ্যাক্টিভিটিতে ফিরে যাওয়া।
AAOS-এর মতে, নির্বাচিত কিছু রোগীর ক্ষেত্রে হাঁটুর কার্যক্ষমতা, অ্যাক্টিভিটি ডিমান্ড এবং ব্যক্তিগত পছন্দের ওপর ভিত্তি করে নন-অপারেটিভ ট্রিটমেন্ট উপযুক্ত হতে পারে।
কখন ACL রিকনস্ট্রাকশন বিবেচনা করা হতে পারে?
রোগীর যদি নিয়মিত ফাংশনাল ইনস্ট্যাবিলিটি থাকে, তিনি যদি হাই-ডিমান্ড পিভটিং স্পোর্টসে ফিরতে চান, হাঁটুর সঙ্গে উল্লেখযোগ্য অন্য ইনজুরি থাকে অথবা শুধুমাত্র রিহ্যাবিলিটেশনের মাধ্যমে প্রয়োজনীয় কার্যক্ষমতা অর্জন সম্ভব না হয়, তাহলে ACL রিকনস্ট্রাকশন বিবেচনা করা হতে পারে।
সম্পূর্ণ ক্লিনিক্যাল পরিস্থিতি বিবেচনা করে অর্থোপেডিক স্পেশালিস্টের সঙ্গে আলোচনা করে এই সিদ্ধান্ত নেওয়া উচিত।
তীব্র isolated ACL টিয়ারের ক্ষেত্রে যখন রিকনস্ট্রাকশন প্রয়োজন বলে সিদ্ধান্ত নেওয়া হয়, তখন অপ্রয়োজনীয় বিলম্ব না করার বিষয়টি বিবেচনা করা হয়, কারণ দীর্ঘ সময় হাঁটু অস্থির থাকলে মেনিস্কাস বা কার্টিলেজে অতিরিক্ত ইনজুরির ঝুঁকি বাড়তে পারে।
সবচেয়ে গুরুত্বপূর্ণ বিষয় হলো, ACL চিকিৎসার সিদ্ধান্ত ব্যক্তিভিত্তিক হওয়া উচিত। শুধুমাত্র এমআরআই রিপোর্ট দেখেই সার্জারি বা নন-সার্জিক্যাল চিকিৎসা নির্ধারণ করা উচিত নয়।
ACL ইনজুরিতে ফিজিওথেরাপির ভূমিকা
ACL ইনজুরির চিকিৎসা নন-অপারেটিভ হোক অথবা ACL রিকনস্ট্রাকশনসহ হোক, উভয় ক্ষেত্রেই ফিজিওথেরাপি রিহ্যাবিলিটেশনের গুরুত্বপূর্ণ ভূমিকা রয়েছে।
রোগীর অ্যাসেসমেন্ট, উপসর্গ, ফাংশনাল লিমিটেশন, রিকভারির স্টেজ এবং ভবিষ্যতে তিনি কোন অ্যাক্টিভিটিতে ফিরতে চান, তার ওপর ভিত্তি করে রিহ্যাবিলিটেশন প্ল্যান তৈরি করা উচিত।
১. ব্যথা ও ফোলা কমানো
প্রাথমিক পর্যায়ে হাঁটুর ফোলা নিয়ন্ত্রণ, ইনজুরিগ্রস্ত অংশকে সুরক্ষা দেওয়া এবং ধীরে ধীরে আরামদায়ক মুভমেন্ট ফিরিয়ে আনার ওপর গুরুত্ব দেওয়া হয়।
হাঁটুতে দীর্ঘ সময় ফোলা থাকলে কোয়াড্রিসেপস মাসলের কার্যক্ষমতা কমে যেতে পারে এবং হাঁটুর স্বাভাবিক মুভমেন্ট বাধাগ্রস্ত হতে পারে।
২. হাঁটুর রেঞ্জ অব মোশন ফিরিয়ে আনা
হাঁটু পুরোপুরি সোজা করার সক্ষমতা বা ফুল নি এক্সটেনশন ফিরে পাওয়া বিশেষভাবে গুরুত্বপূর্ণ।
এরপর উপসর্গ, টিস্যু হিলিং এবং অন্য কোনো ইনজুরি বা সার্জিক্যাল রেস্ট্রিকশন অনুযায়ী ধীরে ধীরে নি ফ্লেক্সন বাড়ানো হয়।
৩. কোয়াড্রিসেপস স্ট্রেংথ ফিরিয়ে আনা
ACL ইনজুরি এবং ACL রিকনস্ট্রাকশনের পর কোয়াড্রিসেপস দুর্বলতা খুবই সাধারণ।
প্রগ্রেসিভ স্ট্রেংথেনিং হাঁটুর কন্ট্রোল, হাঁটা, সিঁড়ি ওঠানামা, দৌড়ানো এবং পরবর্তী পর্যায়ে খেলাধুলার জন্য প্রয়োজনীয় সক্ষমতা ফিরিয়ে আনতে সাহায্য করে।
৪. হ্যামস্ট্রিং, হিপ ও লোয়ার লিম্ব শক্তিশালী করা
ACL রিহ্যাবিলিটেশন শুধু একটি মাসলকে কেন্দ্র করে করা উচিত নয়।
হ্যামস্ট্রিং, গ্লুটিয়াল মাসল, কাফ এবং পুরো লোয়ার লিম্ব কাইনেটিক চেইন হাঁটুর কন্ট্রোল ও ফাংশনে ভূমিকা রাখে। তাই রোগীর রিহ্যাবিলিটেশন স্টেজ অনুযায়ী এক্সারসাইজ এবং লোড প্রগ্রেশন করা হয়।
৫. ব্যালেন্স ও নিউরোমাসকুলার কন্ট্রোল উন্নত করা
ব্যালেন্স ও প্রোপ্রিওসেপশন ট্রেনিং ইনজুরিগ্রস্ত পায়ের ওপর নিয়ন্ত্রণ পুনরুদ্ধারে সাহায্য করে।
প্রাথমিক ওয়েট-বেয়ারিং কন্ট্রোল থেকে ধীরে ধীরে সিঙ্গেল-লেগ টাস্ক, ডায়নামিক ব্যালেন্স, ল্যান্ডিং এবং চেঞ্জ-অব-ডিরেকশন ট্রেনিং পর্যন্ত এক্সারসাইজ প্রগ্রেস করা যেতে পারে।
৬. ফাংশনাল মুভমেন্ট পুনরুদ্ধার
রিহ্যাবিলিটেশনের লক্ষ্য রোগীকে তার দৈনন্দিন প্রয়োজনীয় কাজ ও শারীরিক কার্যক্রমে ফিরে যেতে প্রস্তুত করা।
রোগীর লক্ষ্য অনুযায়ী এর মধ্যে থাকতে পারে:
- স্বাভাবিকভাবে হাঁটা
- সিঁড়ি ওঠানামা
- স্কোয়াট করা
- দীর্ঘ সময় দাঁড়ানো
- পেশাগত কাজ
- জগিং ও রানিং
- জাম্পিং ও ল্যান্ডিং
- কাটিং ও পিভটিং
- স্পোর্টস-স্পেসিফিক অ্যাক্টিভিটি
৭. নিরাপদ রিটার্ন-টু-স্পোর্টের প্রস্তুতি
ACL ইনজুরি বা সার্জারির পর খেলাধুলায় ফেরার সিদ্ধান্ত শুধু কত মাস হয়েছে তার ওপর নির্ভর করা উচিত নয়।
স্ট্রেংথ, রেঞ্জ অব মোশন, হাঁটুর ফোলা, ফাংশনাল পারফরম্যান্স, মুভমেন্ট কোয়ালিটি, স্পোর্টস-স্পেসিফিক ক্যাপাসিটি এবং সাইকোলজিক্যাল রেডিনেসও বিবেচনা করা জরুরি।
আধুনিক ACL রিহ্যাবিলিটেশনে শুধু নির্দিষ্ট টাইমলাইন অনুসরণ না করে ইন্ডিভিজুয়ালাইজড এবং ক্রাইটেরিয়া-বেসড প্রগ্রেশন-এর ওপর বেশি গুরুত্ব দেওয়া হয়।
ACL রিহ্যাবিলিটেশনের স্টেজ
প্রতিটি রোগীর রিহ্যাবিলিটেশন প্ল্যান আলাদা। তবে সাধারণভাবে ACL রিহ্যাবিলিটেশনকে কয়েকটি স্টেজে বোঝানো যায়।
স্টেজ ১: হাঁটুকে সুরক্ষা দেওয়া এবং বেসিক ফাংশন ফিরিয়ে আনা
এই পর্যায়ের লক্ষ্য হতে পারে:
- ব্যথা ও ফোলা নিয়ন্ত্রণ।
- নি এক্সটেনশন পুনরুদ্ধার।
- ধীরে ধীরে নি ফ্লেক্সন বাড়ানো।
- কোয়াড্রিসেপস অ্যাক্টিভেশন উন্নত করা।
- নিরাপদ ও স্বাভাবিক হাঁটার প্যাটার্ন ফিরিয়ে আনা।
স্টেজ ২: স্ট্রেংথ ও মুভমেন্ট পুনর্গঠন
উপসর্গ কমে আসার সঙ্গে সঙ্গে রিহ্যাবিলিটেশন আরও অ্যাক্টিভ হয়।
এই পর্যায়ে কাজ করা হয়:
- কোয়াড্রিসেপস ও হ্যামস্ট্রিং স্ট্রেংথ।
- হিপ ও কাফ স্ট্রেংথ।
- ব্যালেন্স ও সিঙ্গেল-লেগ কন্ট্রোল।
- স্কোয়াট ও স্টেপিংয়ের মতো ফাংশনাল মুভমেন্ট।
- দৈনন্দিন কাজের সক্ষমতা বাড়ানো।
স্টেজ ৩: অ্যাডভান্সড স্ট্রেংথ ও ডায়নামিক কন্ট্রোল
পরবর্তী পর্যায়ে রিহ্যাবিলিটেশন প্রগ্রেস হতে পারে:
- হাই-লেভেল স্ট্রেংথ ট্রেনিং।
- সিঙ্গেল-লেগ লোডিং।
- ল্যান্ডিং মেকানিক্স।
- প্লাইওমেট্রিক প্রস্তুতি।
- রানিংয়ের প্রস্তুতি।
- আরও চ্যালেঞ্জিং ব্যালেন্স ও কো-অর্ডিনেশন এক্সারসাইজ।
স্টেজ ৪: রানিং, অ্যাজিলিটি ও স্পোর্টস-স্পেসিফিক রিহ্যাবিলিটেশন
যেসব রোগীর লক্ষ্য খেলাধুলায় ফিরে যাওয়া, তাদের ক্ষেত্রে ধীরে ধীরে যোগ করা হয়:
- রানিং প্রগ্রেশন।
- অ্যাক্সেলারেশন ও ডেসেলারেশন।
- জাম্পিং ও ল্যান্ডিং।
- চেঞ্জ-অব-ডিরেকশন ড্রিল।
- অ্যাজিলিটি ট্রেনিং।
- স্পোর্টস-স্পেসিফিক মুভমেন্ট প্যাটার্ন।
স্টেজ ৫: রিটার্ন-টু-স্পোর্ট অ্যাসেসমেন্ট
আনরেস্ট্রিক্টেড স্পোর্টসে ফেরার আগে ফিজিওথেরাপিস্ট মূল্যায়ন করতে পারেন:
- মাসল স্ট্রেংথ।
- হপিং বা জাম্পিং পারফরম্যান্স।
- মুভমেন্ট কোয়ালিটি।
- রানিং মেকানিক্স।
- হাঁটুর বর্তমান উপসর্গ।
- সাইকোলজিক্যাল রেডিনেস।
হাই-ডিমান্ড স্পোর্টসে ফেরার সিদ্ধান্তে শুধু সময় নয়, একাধিক ফিজিক্যাল ও সাইকোলজিক্যাল ক্রাইটেরিয়া বিবেচনা করা গুরুত্বপূর্ণ।
ACL রিকনস্ট্রাকশনের আগে ফিজিওথেরাপি
ACL সার্জারি করার সিদ্ধান্ত নেওয়া হলেও অপারেশনের আগে ফিজিওথেরাপি গুরুত্বপূর্ণ হতে পারে।
এই পর্যায়কে সাধারণত প্রি-অপারেটিভ রিহ্যাবিলিটেশন বা প্রিহ্যাবিলিটেশন বলা হয়।
প্রিহ্যাবিলিটেশনের লক্ষ্য:
- হাঁটুর ফোলা কমানো।
- ফুল নি (Full Knee) এক্সটেনশন ফিরিয়ে আনা।
- নি ফ্লেক্সন (Knee flexion) উন্নত করা।
- কোয়াড্রিসেপস অ্যাক্টিভেশন ও স্ট্রেংথ উন্নত করা।
- হাঁটার প্যাটার্ন উন্নত করা।
- পোস্ট-অপারেটিভ রিহ্যাবিলিটেশন সম্পর্কে রোগীকে প্রস্তুত করা।
বর্তমান রিহ্যাবিলিটেশন গাইডলাইন অনুযায়ী, প্রি-অপারেটিভ রিহ্যাবিলিটেশন পোস্ট-অপারেটিভ পর্যায়ে কোয়াড্রিসেপস মাংসপেশির শক্তি এবং হাঁটুর রেঞ্জ অব মোশন উন্নত করতে সহায়তা করতে পারে। পাশাপাশি এটি রোগীকে সামনে থাকা রিহ্যাবিলিটেশন প্রক্রিয়া সম্পর্কে আগে থেকেই বুঝতে ও প্রস্তুত হতে সাহায্য করে।
ACL রিকনস্ট্রাকশনের পর ফিজিওথেরাপি
ACL রিকনস্ট্রাকশন সার্জারিই রিকভারির শেষ ধাপ নয়। হাঁটুর কার্যক্ষমতা ফিরিয়ে আনা এবং ধীরে ধীরে বেশি ফিজিক্যাল ডিমান্ডে ফিরতে স্ট্রাকচার্ড পোস্ট-অপারেটিভ রিহ্যাবিলিটেশন প্রয়োজন।
প্রাথমিক পর্যায়ে সাধারণত গুরুত্ব দেওয়া হয়:
- হাঁটুর ফোলা নিয়ন্ত্রণ
- রেঞ্জ অব মোশন
- কোয়াড্রিসেপস অ্যাক্টিভেশন
- সার্জনের নির্দেশনা অনুযায়ী প্রগ্রেসিভ ওয়েট-বেয়ারিং
পরবর্তী পর্যায়ে ধীরে ধীরে স্ট্রেংথ, ব্যালেন্স, পাওয়ার, রানিং, জাম্পিং, চেঞ্জ অব ডিরেকশন এবং স্পোর্টস-স্পেসিফিক প্রস্তুতির ওপর গুরুত্ব বাড়ে।
রিহ্যাবিলিটেশনের প্রগ্রেশন রোগীভেদে আলাদা হওয়া উচিত। গ্রাফট হিলিং, মেনিস্কাস রিপেয়ারের মতো অন্য কোনো সার্জিক্যাল প্রসিডিউর, বর্তমান উপসর্গ এবং অবজেক্টিভ রিহ্যাবিলিটেশন ক্রাইটেরিয়াও বিবেচনা করতে হয়।
ACL ইনজুরির পর বাসায় এক্সারসাইজ করা যায় কি?
